How to move in

Choosing a care home is overwhelming, especially when it's happening during a crisis — a hospital discharge, a fall, a sudden change in health. We built our admissions process to be calm, clear, and free of pressure. Here's exactly what to expect, from the first phone call to move-in day.


1. Initial Conversation

Call or send us a message. In a 15-minute phone call, we'll discuss your loved one's care needs, answer basic questions about Laurel Ridge, and determine whether a tour makes sense. There is no cost and no commitment at this stage.


2. Private Tour

We schedule a private tour at a time that works for you. You'll walk through the home, meet Sabrina, taste something from our kitchen, and see where your loved one would actually live. We'll talk about daily routines, our care philosophy, pricing, and any specific needs. If you'd like, you can reserve a room with a refundable deposit at the end of the tour.


3. Move-In Preparation

In the days leading up to move-in, we coordinate the logistics that often get missed — transferring prescriptions to our pharmacy, obtaining current physician orders, confirming any assistive devices or durable medical equipment, and preparing the resident's room. We work directly with hospitals, skilled nursing facilities, or home care teams to ensure a smooth handoff.


4. Welcome Home

On move-in day, we focus on one thing: making your loved one feel at home. We help with unpacking, introduce routines slowly, and stay in close communication with family during the first 72 hours — the hardest part of any transition. Within 30 days, we complete and sign a personalized Negotiated Care Plan that guides every detail of ongoing care.

To make the paperwork easier when you are ready to move forward, here is what we will eventually need:

  • Current physician orders for any assistive devices (wheel chair, cane, walker)

  • Current physician orders and a list of all medications including over the counter, prescribed, and supplements

  • Current insurance cards and identification cards

  • Advance directives (POLST, Living Will, Power of Attorney or Durable Power of Attorney for Healthcare) if available

  • Emergency contact information for family and healthcare providers

  • TB screening results within the last 12 months
